How to Complete It — and Why It Matters
Before your Intro Call, we ask you to complete a short questionnaire. This helps your Advisor begin reviewing your financial structure and gives your Integration Specialist the head start they need to connect your GL — so the Intro Call is a working session, not a data collection meeting.

Before You Start — Have These Ready
Gather these files before opening the form. Uploading them is the most time-consuming step.

| Chart of Accounts | Export from your GL: all active and inactive accounts with account numbers, names, and descriptions. Include all segments (Department, Location, Division, etc.) |
| Budget File | Your most recent budget in Excel — account numbers and values by period. |
| Income Statement | Excel format with account numbers, roll-ups, and values. |
| Balance Sheet | Excel format with account numbers, roll-ups, and values. |
| Trial Balance | Last closed fiscal year, Excel format with account numbers and values. |
| Company Calendar | Fiscal year calendar by month with period start and end dates. |
| ✓ Tip: All file uploads must be in Excel (.xlsx or .xls). PDF or CSV exports will not work. |
The Form — Section by Section
The questionnaire has 6 pages. Here is what each one asks and why.
Page 1 — Contact Info
Your name and email address. This is how we associate the submission with your account.
Page 2 — Data & Supporting Files
This is the most important page and the reason we recommend gathering your files first. You will upload:
- Chart of Accounts (active and inactive accounts + desired GL segments)
- Budget File (most recent, by period)
- Income Statement (with account numbers and roll-ups)
- Balance Sheet (with account numbers and roll-ups)
- Trial Balance (last closed year)
Your Advisor uses these files to understand your financial structure before the Intro Call. Your Integration Specialist uses them to plan the GL connection. The more complete these are, the more productive your first session will be.
Page 3 — GL / ERP Information
Configuration details about your General Ledger system. Your IT or ERP admin may be the best person to help with this page. Questions include:
- Number of companies in your GL
- GL segments you want imported into Centage (e.g., Department, Location, Division)
- Whether you use custom or user-defined fields in your ERP
- Whether you need multi-company consolidation
- Your Retained Earnings account number
- Number of currencies (and which ones, if more than one)

Page 4 — Company Calendar
Your fiscal year setup. This ensures Centage aligns to your planning periods correctly.
- Upload your fiscal calendar (start and end date of each period)
- Calendar type — e.g., Jan–Dec, July–June
- Whether your fiscal year is based on year start or year end (for off-calendar companies)
- Number of months in your fiscal calendar
- When you typically start your budget cycle
Page 5 — General Info
A short section to help your Advisor understand your context and priorities.
- Your industry
- Which Centage Advisor you have been assigned
- Your top 3 goals with Centage
- The single biggest pain point you want Centage to solve
These answers shape how your Advisor prepares for the Intro Call. Take a moment with them — they are worth it.
Page 6 — Training Contacts
Who will be trained on Centage and who handles IT support. You will need:
- Primary training contact (name, email, phone, time zone)
- IT support contact (name, email, phone, time zone)
- Full list of trainees and their email addresses
A Few Tips
| Situation | Recommendation |
| Your files are not ready | Gather them first. Do not open the form until you have your Chart of Accounts, Budget, Income Statement, Balance Sheet, and Trial Balance on hand. |
| You are unsure about GL details | Bring in your ERP admin or IT contact before Page 3. Their input will save you time. |
| You get interrupted mid-form | The form does not auto-save. If you need to stop, note where you are and plan to restart from the beginning. |
| Something looks wrong in your data | Submit what you have and note the issue in the pain point field on Page 5. Your Advisor will work through it with you. |
| You have questions | Reply to your welcome email before filling out the form. Someone from the team will respond the same day. |
